Can you please be more verbose about the version requirements?  Please
note that new packages are builded against the unstable distribution
(=sid).  The versions in current *stable* release are not really
relevant for a new package according to an upload to the Debian pool.
(For sure I perfectly know that this is a very relevant question for
your personal setup.  However this has to be handled via backports.) So
if there are outdated versions of predependencies for your package you
should file bug reports against these packages to push the maintainers
for an update.

Yes.  Just put the comments in the beginning of the patch diff file.
Quilt (and patch) are clever enough to ignore this.

Make sure to set the versions in your (Build-)Depends properly to
reflect your requirements.
