I just fixed a similar bug in insighttoolkit (it's as simple as
including <stddef.h>).  I can prepare and upload gdcm 2.0.17-3 with a
patch to do so.  Before doing so, I wanted to check whether you want
to take a different strategy with this, e.g. new upstream.
