  * rules: Reworked CMAKE_FLAGS from scratch.  Update for new install
    practice: libs in /usr/lib, includes in /usr/include/ITK-4.0.

  * patches/gzgetc.patch: New.  Fix to build with zlib where gzgetc is a
    macro.
