[Debian-med-packaging] Requesting upload

Hi,

I've just corrected a severe bug in the freemedforms-project upstream
code and just released v0.9.4.
Debian code is unchanged (but debian/changelog).
Can you please upload the package.

PS: there should be a lintian typo warning about informationS in the
libUserManager.so please ignore it.
