If this is still current: You can easily produce one from your DICOM
files using DCMTK's dcmgpdir:
dcmgpdir [dcmfile-in...]
Note that file names must conform to ISO 9660 Level 1, i.e. only consist
of uppercase letters, numbers, underscore and a maximum of 8 characters
with no file extension. For dcmgpdir, the files should be uncompressed,
i.e. should be encoded with one of DICOM' uncompressed transfer syntaxes.
