There is a ticket to move Sage to Cython 0.29:
https://trac.sagemath.org/ticket/25292
It does require a few trivial adjustments to Sage to compile properly,
because of changes to the PyLong interface.
There are planned follow-up tickets which will make more serious use of
Cython 0.29. So it would be good to know to what extent it will be
problem to rely on Cython 0.29.
