xsltproc warned me that my DocBook/XML documents are using a deprecated
XInclude namespace (http://www.w3.org/2001/XInclude/), so I updated the
XML to use the suggested namespace URI (http://www.w3.org/2003/XInclude).
As a result, xsltproc now crashes with a segmentation fault :(
