it seems this comes from:
/usr/share/xml/docbook/stylesheet/nwalsh/fo/param.xsl
provided by docbook-xsl

the bug is not triggered if you use:
xmlto --extensions

perhaps setting title.margin.left in a xsl applied with
xmltool -m works, too
