That was not the only conclusion, I followed up to that with an alternative,
which was to create a customized DSSSL stylesheet:

You could also submit an enhancement for the docbook-dsssl stylesheets to add
the affiliation element to the variable article-titlepage-recto-elements.

IMHO, this is not a docbook-utils bug, but that's the maintainer's call.
