Ok, I edited the prerm and then dist-upgraded again; as expected, it
didn't attempt to run the prerm script because docbook-xml had already
been unpacked.

I then did an 'apt-get install docbook-xml=4.4-5', installing the version
from etch; there were no errors (and the rest of the packages that
depended upon docbook-xml finished configuring).

I edited 4.4-5's prerm script (removing the ||true's), and did another
dist-upgrade; docbook-xml upgraded from 4.4-5 to 4.5-5, with no errors.

At this point, I fear that I can't reproduce it.  Considering that
you can't reproduce it in a chroot, I suspect it can only be
reproduced on a fresh etch install with a dist-upgrade to lenny.
