Consider as a simple testcase test.xml:

<?xml version="1.0" standalone="no"?>
<!DOCTYPE book PUBLIC "-//OASIS//DTD DocBook XML V4.2//EN"

I get:

$ /usr/bin/docbook2x-texi test.xml
error : xmlAddEntity: invalid redeclaration of predefined entity

"xmllint --loaddtd --valid test.xml" does not show any redeclaration
of predefined entity issue. So this seems to come from docbook2x-texi.
