Fix for reverse_iterator and previous patches

Hi,

The attached patch compiles with Gcc-4.3, contains the patch from
Martin Shreiber, the patch from Paul Harris, and the correct fix -
this time - for the reverse iterator.

The patch is to be applied on a clean master checkout. Please test it,
since the fix for the reverse_iterator is rather intrusive. If no bug is found
within the next couple of days, I'll push the patches to the master and
release a new version of the library.
