Bug fix for uninitialized memory

_M_header is now an attribute of kdtree declared as a _Node_Base.
_M_root is now correctly declared as a _Link_type.

Both test_kdtree and test_hayne pass.

Patch is to be applied on top of paulharris/bugfixes branch or just
checkout sb/bugfixes.
