On my system, the bug is kicking and !screaming.
Unfortunately, I did not note when it broke, .

Attached are the outputs of alsa-info.sh from a Squeeze system where it still 
works and the current non-working system.

To order the builtin and HDMI sound, I have
    # grep -v \# /etc/modprobe.d/alsa-base-local.conf
    options snd cards_limit=2
    options snd-intel8x0 index=0
    options snd-hda-intel index=1


Don't know if it helps, but when I attempted to fix the problem last november, 
one of the things I tried was the recipe from 
http://felimwhiteley.wordpress.com/2010/03/30/fixing-fat-fingered-audio-
output-device-deletion-in-kde, which did not help in itself (no sound from 
aplay). But on logout, KDE played the shutdown sound. Unfortunately, this was 
the only sound ever since, reapplying the recipe did not help anymore.
