Unless it has been fixed *very* recently, this bug (boot fails to ask for password of second physical volume of VG containing root) has 
not been fixed.
(At least) part of the problem may be in /usr/share/initramfs-tools/hooks/cryptroot, in the get_lvm_deps function where it calls 
"dmsetup deps" to get the major/minor device ids of the dependancies of the VG containing root.  At least on my (current Lenny) system, 
"dmsetup deps" returns only a single pair of numbers, even though there are two PVs underlying that VG.
I'm not knowlegable enough in the guts of LVM2 to provide a patch, but I hope this helps.

PS:  If this bug was supposed to be fixed in Lenny, maybe it would be appropriate to raise it's priority to something above "wishlist"?
