please provide more information about your setup, the used versions,
etc. do you use evms? does evms have a initramfs hook which installes
the evms binary into the initramfs? what is the exact output of
'update-initramfs -u'?

and please attach initramfs.log after executing 'sh -x mkinitramfs -o
/tmp/initramfs.debug 2>initramfs.log'
