This looks like you got bitten by the change to the /etc/mailname
handling which we introduced a few versions ago. Please see the
NEWS.Debian file.
If you can still reproduce the bug, please put a message on the queue,
and deliver a typescript of exim4 -d -M $MESSAGE_ID which shows a
failed delivery attempt.
