Bug#482404: exim4-daemon-heavy: SSL issue with Outlook still not resolved

I'm still seeing this mysterious issue - can't send mail using Outlook
2003 (port 465, SSL on connect, SMTP-AUTH).

Despite having these lines at the top of the configuration:

MAIN_TLS_VERIFY_CERTIFICATES = /dev/null
MAIN_TLS_VERIFY_HOSTS =
MAIN_TLS_TRY_VERIFY_HOSTS =

which should avoid the "too many certificates" bug, it still fails
with the same "A TLS packet with unexpected length was received".
It has been working for a few years, probably stopped working about
the time of etch->lenny upgrade, and now I'm trying the latest exim4
from lenny-backports, but the issue still exists.  Any suggestions?
