Thanks again Andreas. Unfortunately I am still having trouble. My 
/etc/exim4/exim4.conf/localmacros looks like this;

$ cat /etc/exim4/exim4.conf.localmacros
MAIN_TLS_ENABLE = true

Have I forgotten a step?
