[Pkg-exim4-users] Exim4 delivery question

I have an annoying message in the mainlog that I don't understand.
no immediate delivery: more than 10 messages received in one
connection


In /etc/exim4/exim4.conf.template I added 
smtp_accept_max_per_connection = 100 as per the exim docs but keep
getting the error. No error message shows up when I run
update-exim4.conf, but the 10 message limit persists.

Is there a special section that the accept should be in?  I have tried
2-3 different sections with no success.

As a side note, my ISP claims to support LOGIN, PLAIN and CRAM_MD5 logins:

But if I have mutt configured with "set reply_to=ask-yes" and "set
followup_to" I get this error:

host smtp.intergate.com [216.139.64.8]: 550 5.7.1 
<pkg-exim4-users-request@lists.alioth.debian.org>... Relaying denied. 
Proper authentication required.

Unsetting reply-to and followup-to seems to allow 'most' mail to get
passed on.

That's when I use LOGIN.  They don't accept cram_md5, at all!

cram_md5 authenticator failed H=smtp.intergate.com [216.139.64.8] 535 5.7.0
authentication failed.

I feel that its a improper setup at the ISP.  I sent message headers showing
that I was 'not' relaying and asked what was up.  They have not answered and it
has been a week since I sent the message.

Odd the for the first month I switched to this ISP I did not have any mail
rejected.  I was using exim3 and was not aware that I needed to authenticate.
After 6 weeks I noticed mail wasn't getting to the list.  That's when i finally
switched to exim4

Any and all suggestions would be appreciated.
