passwd.client is used when exim is acting as client, i.e. when exim
takes a messages and passes it on to another server that requires exim
to show some authentication.

You OTOH want exim to request authentication from clients connecting
to it. You'll need to enable at least one authenticator in exim's
configuration file (plain_server, as Outlook afaik does not support
CRAM-MD5), possily enable TLS/SSL and fill /etc/exim4/passwd.
