It sounds as if that configuration may be something you're using
instead of the one provided in the exim4-config package. But if
you're using the latter, there is a spot commented in the config for
dropping in your content scanner.
I think any kind of content scanning requires exim4-deamon-heavy
(unless you go with sa-exim and don't use a virus scanner).
