Having database lookups in expanded options is not really a supported
szenario, iirc. Overriding the macro by adding

MAIN_RELAY_NETS=: 127.0.0.1 : ::::1 : net-dbmnz;/var/lib/pop-before-smtp/hosts.db

to /etc/exim4/exim4.conf.localmacros should work, though.
