Add English CD version of The Last Crusade

Ever vigilant :-). I'm pretty sure my CD isn't scratched, I imaged it 
with cdrdao and it didn't report any errors.

Anyway, the only differences between the two versions are the default 
save file name ("savegame.___" in the floppy version, "C:\INDY3.___" in 
the CD version, which makes sense) and the text explaining how to buy 
the hint book which is removed in the CD version (which came with the 
hint book in the box).
