Did you try it out with various codecs ? Please disable all codecs in the
preferences and try them enabling one by one and see if you have the
problem in all cases or only in with some of them.

You don't need a webcam to use the phone communication. Have you had
success with others sound acquisition device?
You also mention "h323" only, what are the results with SIP? Sip is the
norm nowadays...
