     1) Loaded glade (fresh blank project was opened by default)
     2) added a new window
     3) added a toolbar
     4) right clicked on toolbar and clicked "Edit"
     5) switched to heirarchy pane
     6) clicked on "Add" button.

   * What exactly did you do (or not do) that was effective (or
     ineffective)?

     N/A

   * What was the outcome of this action?

     Segfault

   * What outcome did you expect instead?

     No segfault .. add to heirarchy as expected.
