Hi Jeremy,

unfortunately you didn't change anything in the package, so I have to reject it again.

The LGPL-3 incorporates the terms and conditions of version 3 of the GNU General Public License.
According to GPL-3 5a: "The work must carry prominent notices stating that you modified it (...)".
So yes, your patch changed the original work and this must be mentioned, thus it must be added to debian/copyright.

Somebody created the files under debian/*, so somebody does hold the copyright.
Given that it was created by Heather using a business email address I would assume that Canonical is the coypright holder.
I am pretty sure that Canonical did not transfer the copyright to the original author of the software.
According to Debians policy 12.5 the copyright information need to be documented in debian/copyright.
