Do you mean a GNOME Shell extension? The gnome-system-monitor package
doesn't contain a GNOME Shell extension, only a standalone application
(/usr/bin/gnome-system-monitor, "System Monitor" in the apps menu).
There is a packaged GNOME Shell extension related to
gnome-system-monitor in the gnome-shell-extension-system-monitor
package, which is part of the gnome-shell-extensions source package.

This is not an extension that is provided by Debian. Probably you
installed it from <https://extensions.gnome.org/> or Github? Debian is
not responsible for third-party extensions.
This might be
<https://github.com/mgalgs/gnome-shell-system-monitor-next-applet/issues/143>.
