debian/changelog should include every change to packaging like `git
log`, it should document the (relevant) changes since the previous revision.

When for example a patch is added, this is recorded in the changelog.
But when that patch is dropped before a package revision was uploaded
that includes it (e.g. because it's included/applied upstream), the
changelog entry about adding the patch should be removed instead of
adding another changelog entry to document its removal.

I've updated the changelog to remove such cases that I could easily
identify. Please go over your changes and remove any further obsolete
entries where possible.
