Bug#395019: unhandled numbering scheme transition

We forgot that hard disk numbering scheme has changed in 1.95 and:

  - update-grub wasn't updated accordingly.

  - Even if it were, the version of update-grub we're running could be out of
    sync with the grub version in boot record.  We need to prompt the user to
    take care of this.
