Thanks to you all.  Colin Watson has found two severe bugs in my assembly
implementation of grub_vbe_bios_getset_dac_palette().  I believe these are
the cause of your bug.

Here's a fixed build.  Please confirm it's working and we'll get this fix
merged ASAP.
