I have also experienced this bug. Kudos to the OP for his hint for fixing it.
Please consider raising severity, as this leads to an unbootable system.
I encountered one similarity: My root is also on /dev/mapper/, but not using LVM.
(crypto fs) /boot is on /dev/sda1.
At least after fixing by reinstalling grub, I do have a stage2. (dated Oct 17, 2009)
