Bug#544731: missing "insmod lvm" in grub.cfg ("error: no such disk")

Adding '(lvm-system) /dev/mapper/lvm-system' you make a pretty unusual
claim that your BIOS has LVM support (or that you're booting in VM and
this LVM partition is in fact VM disk)
Another part of your bugreport looks like a problem with LVM module.
Some of them were fixed recently (including snapshot problem). Please retest
