Are there any messages on the screen?

Anyway, this probably means that your system is misconfigured and is
installing GRUB to somewhere that doesn't correspond to where your
system actually boots from, resulting in the core image and modules
being incompatible.  Please post the output of:

  debconf-show grub-pc
  cat /proc/mounts
  ls -l /dev/disk/by-id/

(You'll need to chroot into the installed system before running at least
the first of those.)
