AFAIK it may affect any version upgrade. It should be less frequent with 
the widespread use of a monolithic EFI signed image on architectures 
which provide it.

Definitely. Or user error such as running grub-install without 
--removable or --force-extra-removable on a system which relies on the 
removable media path.
