"people installing nodejs in debian expect npm to be available" is
pretty much the definition of Recommends in policy.

It is not hard to think of use cases for nodejs without npm. To give a
concrete example I use nodejs for teaching, but the students don't need
to install any npm packages. I imagine it also simplifies bootstrapping
to avoid the hard dependency.
