I wonder if that has anything to do with erroneous blanking as described at:
http://bugs.debian.org/cgi-bin/bugreport.cgi?bug=579016
though it is not entirely clear whether libburn has been used here, rather
than dvd+rw-format.

Also, could you please try these, so we better know where the problem hides:
cdrskin -v dev=/dev/XXX blank=format_overwrite
dvd+rw-format --blank=full --force=full
