One thing was missing from that list: libpam-systemd.
I was able to reproduce this by taking a plain jessie vm and installing the following:
- libpam-cgfs
- cgroupfs-mount
- sysvinit-core
- libpam-systemd
- linux-image-4.5.0-0.bpo.2-amd64

Not using kernel 4.5.x does not trigger the issue.

I guess this is an issue somewhere between the new cgroups in 4.5 and systemd-shim.
I'll try to pinpoint that futher when I have some time.
