I did some more tests (again today (*))
first let me give names to the 3 situations
1) 'mplayer -stop-xscreensaver' with mouse in window
2) 'mplayer -stop-xscreensaver -fs' fullscreen
3) 'mplayer -stop-xscreensaver' with mouse outside of window
so, I checked that using
mplayer-nogui 1.0~rc3+svn20090426-2
in Debian/sid (taken from incoming),
all works as I would expect:
1,2 -> no screensaver
3 -> screensaver starts
but instead I found a box Debian/lenny
(with mplayer 1.0~rc2-17+lenny3), and there
1 -> no screensaver
2,3 -> screensaver starts
and the (2) was maybe the bug that Sam reported as 498895
(although he did not mention fullscreen)
so it seems that the bug does not occur anymore... at least in my PC

ps *: the reason why I checked again is that I realized that modern
optical mice are quite sensitive, so any small movement that I do (even
walking around the room) does deactivate the screensaver ... so I
unplugged the mouse and did the experiment again
