test on a clean vps install.
aptitude install mysql-server mysql-client
(then skip/enter password for root. whichever...)
then:
#mysql -uroot -p123
ERROR 1045 (28000): Access denied for user 'root'@'localhost' (using password: YES)
#/etc/init.d/mysql stop
#mysqld_safe --skip-grant-tables &
#mysql -uroot mysql
mysql> SELECT Host, User FROM mysql.user;
only debian-sys-maint on the user list.
no root user!
