[debian-mysql] Bug#682311: mysql-5.5: some missing copyright attributions

Dear Maintainer,

Firstly the file debian/copyright does not contain any copyright attributions for teh debian/* files
which could be deduced from the debian/changelog as the Debian Perl Group does.

Secondly the file debian/additions/mysqlreport has some specific copyright attributions to make.
As does presumably the innotop code.
