It looks like this message is coming from upstream code, in
scripts/mysql_system_tables_fix.sql in the source. It probably needs
checking and forwarding upstream. Without steps to reproduce, I'm not
prepared to do this. Please provide exact steps to reproduce this
problem.

Assuming this only happens if a user is upgrading through from a pre-4.1
version of MySQL, I don't think we can treat this as anything but a
minor bug, and I'm afraid it will have to go to the bottom of the queue.
MySQL 5.1, for example, first shipped in Debian in 2010. To my
knowledge, nobody has complained about this lack of documentation in six
years.

In practice, this means that it will probably not be looked at even with
steps to reproduce, unless someone interested volunteers.

If it turns out that the problem can in fact be reproduced starting from
a modern installation of MySQL, I may of course change my mind.

These are all reasonable questions, but they appear to me to apply to
MySQL users generally, across all distributions. If this is true, the
right place to resolve this would be in upstream's user manual by
forwarding this bug.
