Instead of the /etc/X11/xorg.conf generated by nvidia-xconfig try a
minimal xorg.conf as described in /usr/share/doc/nvidia-glx/README.Debian.gz

Purge and reinstall everything from qt.

Create a new user and retry with all the dotfiles being reinitialized
from scratch - eventually some config file has collected some bitrot
that causes trouble. If that helps, try moving away suspicious
dotfiles/dotdirs from your home, .qt* should be a good start.

How exactly does the crash look like?

* is it only the application, can you get a backtrace?
* does the xserver itself crash, what does /var/log/Xorg.*.log* say?
* can X be restarted afterwards? Is the machine reachable by ssh?
* does the full system lock up, requiring the reset or power button to
be pressed for recovery?
