That would be caused by user/configuration error.  Either they loaded a new 
LDIF with no CSN information, and failed to pass the correct flags to 
slapadd to generate it, or they haven't correctly configured syncprov 
information to store the contextCSN so that it is always available.  I'm 
guessing the former.  I don't see this as a case to account for.
