Same here, both with upstream 4.0.3 and debian's 4.0.3 (both with sqlite as db). 
I will test later for mysql and pgsql, but since it's with upstream, I expect 
the result to be the same.

Simply refreshing in the Files application adds another folder of the same name.
Note that this is a web interface issue only, when using it through webdav I 
only see 1 folder.
