Bug#462775: libcurses-perl: FORMS support missing

The FORMS support in libcurses-perl is currently missing. There's no
note of this in the changelog, but debian/README.Debian states

 This package is built with support for MENUS and PANELS. FORMS support
 does not yet work under Debian.

The problem is probably the one explained in the INSTALL file:
/usr/lib/perl/5.8/CORE/form.h conflicts with /usr/include/form.h from
libncurses5-dev, and the Perl directory is first on the include path.

An easy way to fix this is to use libncursesw5-dev instead (see #462773);
then we have form.h in /usr/include/ncursesw and the directory first on
the include path.

Filing this to document the issue.
