Hm.

My experience: This file is shipped in hundreds of Perl distributions
and Debian source packages, and I've never seen it been re-generated
at build time.

I might be wrong, but its character reminds me of the bunch of
autofoo files that are included all over the place without the need
to rebuild them each time during package build.

IMO being able to generate ppport.h with Devel::PPPort in perl core
should be enough in this case as well.

But others might have more experience and/or different opinions here.
