   * What led up to the situation?

Over time, the location of the association lists has changed. Currently, mimeinfo uses the old ~/.local/share/defaults.list , where the current standard [1] dictates the more simple ~/.config/mimeapps.list. Programs such as Iceweasel use latter as well.
