[pkg-php-pear] Updating php-code-coverage

I’ve pushed the php-code-coverage update to Git, but I’ll take care of
updating php-token-stream (>= 1.3) first to make it installable (I’d like
to test them together anyway). I’d also like to add DEP-8 tests before the
upload.

The php-token-stream package also needs to be rewritten for Composer.

I kept the current libjs-twitter-bootstrap dependency, some features may
thus be missing (the currently packaged version 2 and the actually needed
version 3 are not supposed to be compatible). If these features are
important, we may ship an embedded copy of the needed JavaScript until the
libjs-twitter-bootstrap 3 package is ready, please let me know.
