Hi,
I'm also seeing this problem:
E: source.c: Assertion 'PA_SOURCE_OPENED(s->thread_info.state)' failed at pulsecore/source.c:278, function pa_source_post(). Aborting.

I can however reliably reproduce it:
Whenever I start pulseaudio and currently NO application has opened audio, pulseaudio will NOT start.
When I have e.g. mpd playing music on the alsa device, pulseaudio will start.

Note that I have a rather complex setup, something like this

pulse -> plug -> softvol  -> dmixer -> hw
mpd           -> softvol2 -> ^^^

So I have the impression pulse fails to setup the alsa plugins such as softvol and plug.
